Child & Family Development Child & Family Development

Getting Started

Your first appointment is called the “Intake.” The purpose of the Intake is to gather a detailed history of your child. We use this information to determine the evaluation or treatment recommendations.

Preparing for your first visit

Before your visit, a Pre-Visit email or text will be sent to you, providing you with a private way to give your provider all the information he or she needs for your upcoming visit, and saving you time in the lobby.

Day of your first visit

Remember to bring your insurance card to your appointment.

Carefully read all of our policies and fill out a medical history form below. Please be aware that some of our services may not be covered under your particular insurance benefit plan.

Select and download the medical history form based on your child’s age and bring it with you to your appointment.

Visit our contact page for maps and driving directions to both the Midtown and Pineville office locations.

If you have any questions about Child & Family Development’s general policies, please call us at 704.332.4834 (Midtown) or 704.541.9080 (Pineville).

What's next?

After your Intake, we make recommendations and set clear goals.

Evaluation

When testing is recommended, we customize the Evaluation process. Testing sessions are scheduled and may vary based on your child’s age and the reason for the Evaluation.

Interpretive Parent Conference

Evaluations conclude with an Interpretive Parent Conference (IPC). This is a separate appointment that outlines the outcome of the overall evaluation. Testing results are explained and recommendations/goals are discussed with the parents.

Written Report

Following the IPC, a Written Report is provided. The report is a compilation of all the information provided during the Intake, Evaluation and Interpretive Parent Conference.
