General Policy

Financial Policy
Payment is expected at the time of service. We accept Visa and MasterCard, as well as your personal check. Please communicate with our business staff prior to receiving services if you would like to access your insurance benefits. Not all services are filed to or payable by insurance carriers.

Managed Care Insurance Plans
If we are a participating provider, all copays, deductibles, co-insurance and non-covered services are due at the time of service. Managed care plans are complex and most always require pre-authorization for services. If you participate in a Managed Care Insurance Plan, it is imperative that you and our business office communicate prior to scheduling appointments. In the event that your insurance coverage changes, C&FD must be notified prior to the effective date of the new policy or we cannot provide you with accurate information about coverage and your cost.

Commercial Insurance Plans
We accept assignment of insurance benefits for most commercial plans. However, we do require copayments, deductibles, co-insurance and non-covered charges to be paid at the time of service. Please be aware that some, and perhaps all, of the services provided may not be covered under your particular benefit plan. Eligibility determination of benefits or coverage is never a guarantee of payment. If your insurance company has not paid services in full within 30 days, the balance will automatically be billed to your account.

Medical Records
Copies of all written communication are automatically shared with parents. With written permission, copies of such reports can go to other professionals, including the pediatrician, other physicians, a school, etc. All information is treated confidentially.

Cancellation Policy
Any appointment cancelled with less than 24-hours notice and not rescheduled will result in a Cancellation Fee. Failure to contact the office prior to the appointment time will result in a No Show Fee. Repeated cancellations may result in either forfeiture of permanent appointments or termination of service.

For psychological emergencies after regular business hours, call the CMC Behavioral Health Call Center at 704-444-2400, call 911 or visit a local emergency room.
