Autism Spectrum Disorder evaluations at C&FD

April is Autism Awarness Month!

Three psychologists at Child and Family Development offer Autism Spectrum Disorder (ASD) evaluations and related services.

ASD primarily affects three areas of behavior:

  • communication
  • social interactions or connectedness
  • behavioral rigidity

Because autism spectrum disorders are diagnosed based on behavior, the evaluation includes both formal assessments, as well as structured play and interaction scenarios.  Standardized tests are used for this purpose, and may include:

  • Autism Diagnostic Observation Schedule, Second Edition (ADOS-2)
  • Autism Spectrum Rating Scales (ASRS)
  • Social Communication Questionnaire (SCQ) 
  • Social Responsiveness Scale (SRS)
  • Vineland Adaptive Behavior System-Second Edition (Vineland)
  • Wechsler Intelligence Scale for Children-Fifth Edition (WISC-V)
  • Wechsler Preschool and Primary Scale of Intelligence- Fourth Edition (WPPSI-IV)

The Wechsler scales are widely used and broadly accepted tests of cognitive skills for children with and without developmental delays or learning differences.  The ADOS is a semi-structured assessment that specifically examines those behaviors commonly associated with autism.  Other standardized instruments and informal measures may be used as needed to determine the presence of the behavioral characteristics of autism spectrum disorder (ASD).

Of course, each evaluation is designed to address the particular needs of each child and family. In addition to addressing the impact of behaviors often associated with autism, comprehensive evaluations may include an assessment of cognitive and intellectual skills, educational performance, social/emotional adjustment and independent living skills.

Similar to our Psychological-Educational evaluations, an ASD evaluation will include an Intake appointment with the parents or caregivers, several evaluation sessions, and an Interpretative Parent Conference providing results and recommendations.

The cost of the evaluation depends on several factors, including:

  • age of the child
  • the complexity of the testing
  • the unique needs of each child

Typically, testing is completed over three sessions. At times, two sessions are adequate. Our focus is on providing the best testing environment and ensuring that each child has the time and energy to do their very best. As a part of a multi-disciplinary practice, we can also assist in coordination of care with other therapists, including speech therapists, occupational therapists, physical therapists, educators and a psychiatrist.

Our April 22 Autism Conference in Charlotte will feature Dr. Gary Mesibov, a renowned expert.  This day-long conference is focused on understanding the neurological differences that research has identified between people with ASD and typical learners and then adjusting educational, clinical, and community-based programs to more closely match the learning styles and needs of those with ASD. The presentations will describe some of the learning differences that have been identified in people with ASD (referred to as The Culture of Autism) and show how these can be accommodated by making evidence-based appropriate adaptations in how interventions are created and delivered. Emphasis will be on strategies for teaching skills that will be needed to help those with ASD to achieve the highest quality of life possible and also on reviewing new approaches that are on the horizon.

We have noticed more and more media attention on aging people with autism spectrum disorder and their access to support and care. 

  • Jessica Hoffarth, MS, OTR/L, found this article from Tom Insel, MD, Director of the National Institute of Mental Health (NIMH) about the Autism CARES Act (for Collaboration, Accountability, Research, Education and Support). The law ensures the continued work of the Interagency Autism Coordinating Committee (IACC), but it also brings new opportunities to focus attention on an area of concern for many parents—what happens when their children with autism spectrum disorder (ASD) mature into adulthood? 
  • This month, NBC featured a story titled "You Don't Outgrow Autism..." on Dateline about individuals aging out of access to public and private support programs and the impact it can have on their functional skills and daily life.  
  • The TEACCH program from the University of North Carolina, Chapel Hill spotlighted their supported employment program providing job coaches for autistic adults and T-STEP (TEACCH School Transition to Employment Program) on their website

Children with Autism Spectrum Disorder (ASD) often present with:

  • social communication difficulties

This can include difficulty with back-and-forth conversations or joint play, poor use of eye contact, difficulty maintaining age-appropriate friendships, as well as unusual interests, repetitive behaviors or excessive adherence to routines.

Our psychologists provide comprehensive and timely assessment and treatment for families seeking a diagnosis and support. 

  • Our evaluations begin with a parent interview
  • Diagnostics include intellectual testing, formal assessment of social and communication skills (using a test such as the ADOS-2) and behavioral questionnaires that are completed by parents and teachers.
  • During the Interpretive Parent Conference, the results are reviewed and recommendations for therapy, school or community interventions are shared. 
  • A written report follows in about 4 weeks.

Counseling helps kids with developmental disabilities

March is Developmental Disabilities Awareness Month!  Visit our blog each Friday of this month to read about some of these conditions and how Child and Family Development helps kids, adolescents and young adults reach their potential. 

The Center For Disease Control website offers excellent summaries and resources to learn more about developmental disabilities: http://www.cdc.gov/ncbddd/developmentaldisabilities/facts.html.  

The CDC has information on specific conditions, such as Autism Spectrum Disorder (ASD).  ASD is a developmental disability that causes difficulties with communication, social, and play skills.  These skills are the building blocks for cooperation, perspective taking, and emotional processing and important for academic subjects that require understanding others, like reading comprehension, social studies, and history.  Deficits can range from mild to severe.  

Brandyn Street, Ph.D., is one of three psychologists here who specializes in the assessment and treatment of autism.  Dr. Street asserts that determining a diagnosis of ASD is an important step in deciding which skills need to be targeted for treatment.  If your child has difficulties making friends, sustaining a conversation, playing in an imaginative way, and is inflexible with routines, it is important to find out why and obtain supports to help.

Learning disabilities: 10 warning signs during school years

Our psychologists and educators can help children, teens and young adults with learning disabilities through comprehensive assessment and goal-directed support.  

There are some classic warning signs of a learning disability during the school years:

  1. Oral reading is slow or labored
  2. Reads with substitutions, adds words or guesses at words
  3. Poor decoding skills (not able to properly “sound out a word”)
  4. Poor spelling skills (often individuals with dyslexia will spell words correctly on a spelling test ,but are not able to generalize into other day-to-day writing assignments)
  5. Poor fine motor skills
  6. Poor handwriting
  7. Trouble with recall or retrieval of math facts, especially quick retrieval
  8. Writes or reads letters and/or numbers reversed
  9. Doesn't enjoy reading
  10. Doesn't enjoy writing

Learning disabilities, and specifically dyslexia, impact approximately 20% of the population. The NC and SC public school systems have resources in special education to identify individuals with learning disabilities however, procedures and guidelines are such that most children are not identified with learning disorders until 2nd or 3rd grade and in order to be identified their delays must be significantly impacting their educational performance. Therefore, many children with mild delays simply “fall through the cracks” as their mild delays turn into moderate and severe delays without appropriate intervention. Research indicates the children make the greatest gains in learning to read in grades K through 2nd. Research has also shown that if the reading gap is not remediated by the 3rd grade, it is very hard to close. Therefore, it is imperative that professionals in the medical field collaborate with parents and educators to help identify individuals with learning disabilities.

Talk with a pediatrician or teacher if you have concerns.  They may recommend psycho-educational evaluation.

Learning disabilities: 10 warning signs during preschool years

  1. Late speech development
  2. Late development with learning alphabets letters and sounds (late is considered by 5 to 5 ½ years)
  3. Inconsistent development and learning of the alphabet and sounds
  4. Poor rhyming skills
  5. Avoiding drawing/coloring, pre-writing tasks
  6. Weak fine motor skills
  7. Late established hand dominance
  8. Difficulty with word retrieval (says ummm and thing)
  9. Advanced vocabulary in comparison to development of reading skills

Child psychologists can help alleviate the stress of test taking

Students often worry about taking tests. Elementary school-aged children stress about the Charlotte Mecklenburg Schools End of Grade (EOG) tests and older kids stress about the PSAT and SAT as they prepare for college.  

Anxious feelings can develop prior to test taking, including lack of sleep, lack of appetite, lack of confidence, easy agitation, and irritability.  Thus, students experiencing test anxiety may not be off to such a great start.  While sleep and food are important, proper preparation prior to testing as well as being able to relax your mind and body during testing so optimal performance can be reached are extremely important. 

If your child experiences stressful and anxious feelings about test taking, consider getting help.  A child psychologist helps students experiencing test anxiety through a series of sessions aimed at reducing worry and fear, raising self-confidence, becoming aware of proper study habits and test taking techniques.

Child psychologists teach the ABCs of managing behavior in kids

The Antecedent-Behavior-Consequence (ABC) behavior management method is one of many features of Applied Behaviroal Anlaysis (ABA)

In this method, a behavior is a specific action that a person does that can be observed. 

Example:

Behavior--Tommy calls out in class. (concrete and measurable)

Not a behavior--Julie is disruptive.

In order to change a behavior, you first need to understand the chain of events that determine what will happen with a specific behavior in the future.

A=Antecedent:  Situation that comes BEFORE a behavior.  This sets the stage for a behavior to occur.  This can be an immediate setting event (i.e., parent request) or a long-term setting event (i.e., did not sleep enough the night before).

B=Behavior:  The actual behavior that is observed.  It is important to observe the behavior and determine if your child is having a skill or performance deficit.  A skill deficit is when the child does not have the skill in his/her repertoire.  A performance deficit is when the child has the skill but does not choose to use it.  It is also important to identify the correct behavior and what function the behavior is serving your child.

C=Consequences: What happens immediately following the behavior.  This can occur naturally or be planned.  The type of consequence that follows a behavior will determine if the behavior will occur more frequently or less frequently in the future.

With careful observation and documentation of the ABC's, one can start to better understand the behaviors, the chain of events, and the factors that are contributing to the behavior.
